The weight of adulthood can feel overwhelming. Life becomes a delicate balance of immense responsibilities and unexpected challenges. At Hope’s Destiny, we understand how the pressures of work, relationships, family, finances, and past traumas can take a toll on your mental and emotional well-being. That’s why our Adult Treatment Services are designed to meet the unique needs of individuals aged 18 and older who are experiencing mental health conditions or co-occurring disorders.

Our goal is to provide a safe, respectful, and supportive environment where adults feel seen, heard, and empowered to take meaningful steps toward recovery—no matter where they are on their journey.

Our Levels of Care for Adults

Our continuum of care is designed to meet each adult exactly where they are. Whether someone is just beginning treatment or stepping down from a higher level of care, we offer flexible options that can evolve alongside their progress.

Partial Hospitalization Program (PHP)

Our PHP provides intensive, structured treatment while allowing adults to return home each evening. This level of care runs 5 days a week for 4 to 5 hours per day and includes daily group therapy, individual therapy sessions 2 to 3 times weekly, and weekly family therapy when needed. Comprehensive psychiatric care and medication management are provided throughout the program. PHPs typically last 3–4 weeks, but the duration can be adjusted based on individual progress and goals.

Intensive Outpatient Program (IOP)

IOP offers focused treatment with the flexibility to maintain work, school, or family responsibilities. Adults attend three-hour therapy sessions, 3 to 5 days per week. Treatment includes individual therapy, group therapy, weekly psychiatric check-ins, and optional family sessions. Life skills development and relapse prevention planning are core components. Most clients remain in IOP for 6–8 weeks, with timing adjusted as needed.

Outpatient Program (OP)

Our OP provides ongoing therapeutic support for those transitioning from higher levels of care or who need consistent but less intensive services. Adults typically participate in weekly individual therapy and monthly family or couples counseling. Optional group therapy and medication management services are also available. OP generally lasts 3–6 months but is fully customizable based on client needs.

  • Biopsychosocial Assessment – Every adult begins their journey with a comprehensive biopsychosocial assessment. This evaluation takes a close look at your emotional, psychological, physical, and social history to better understand what you’re going through and how best to support your recovery journey. This helps us create a personalized treatment plan that meets you where you are.
  • Psychiatric Evaluation – If appropriate, you will meet with a licensed psychiatric provider to help clarify any diagnoses and explore whether medication might support your overall treatment. Ongoing monitoring is provided if medication becomes part of your plan.
  • Individual Therapy – Personalized, one-on-one therapy is the cornerstone of our treatment. With the support of a licensed therapist, you’ll explore your challenges, develop practical tools, and build resilience to create lasting change in your life.
  • Group Therapy – Group sessions provide a space for connection, support, and shared healing. Led by experienced clinicians, these groups focus on emotional regulation, relapse prevention, trauma recovery, communication, and more.
  • Family or Couples Therapy – Mental health and substance use challenges often affect more than just the individual. Our family and couples therapy sessions focus on rebuilding trust, improving communication, and creating supportive dynamics at home.
  • Medication Management – When clinically appropriate, our psychiatric team provides evaluation, prescriptions, and ongoing medication monitoring. We collaborate with you to ensure your medication plan is safe, effective, and supportive of your recovery goals.
  • Life Skills Training – Recovery goes beyond symptom management. We help you build essential life skills—from stress management and healthy boundaries to career readiness and financial planning—that support your independence and long-term wellness.

Our case managers help coordinate care, connect you to community resources, and provide ongoing support throughout your journey—ensuring no detail of your recovery is overlooked.
